Application & Forms for Body Artists

The forms below are for use by new and existing body artists/establishments in Cattaraugus County.

The "CCHD: Application for a Certificate to Operate a Tattoo Establishment" is used by new tattoo and permanent makeup establishments who do not already have a "Certificate of Sanitation."

The "CCHD: Consent Form for the Body Piercing of a Minor" is used by body artists to document parental consent when piercing minors.  It is illegal to pierce a minor without parental consent.
