Beginning September 01, 2007 mail the yellow research copies of all of your agencies' completed and voided PCRs to:

Also you can feel free to drop them off to our office at the address above.
PCRs are still to be sent to us by the 10th of the month following the month of the call (ie: all September PCRs should be received by October 10th). Continue to include a summary detailing your agency code, agency name & contact information, the # of new/blank NYS DOH Version 5 PCRs you have received, the number of PCRs used by your agency, and the number of the voided PCRs.
Please let me know also if your agency used a form other than a NYS DOH Version 5 'Paper' PCR or if electronic PCR submission to the state is used; also where you receive your PCRs from.
