In Cattaraugus County, any establishment preparing, handling, and serving food to the public requires a food service permit issued by the Health Department.  Other types of food handling operations may require different licenses from other agencies (see below for more information). Read more

Agricultural Fairgrounds & Mass Gatherings

Agricultural fairgrounds and mass gatherings in Cattaraugus County require a “Permit to Operate” issued by the Health Department.  An agricultural fairground is any property, and associated buildings and structures, where a fair is conducted or proposed by an agricultural society or other eligible entity.  A mass gathering is any event that is likely to attract more than 5000 people and continue for 24 hours or more. Public Swimming/Bathing Facilities

Public swimming facilities in Cattaraugus County require inspection and a “Permit to Operate” issued by the Health Department.  Typically, these fall into one of two categories, swimming pools and bathing beaches.  If you intend to operate one of these facility types, please contact the Environmental Health Division.  Note that all public swimming facilities require review and approval of engineering plans prior to construction. Application & Forms for Body Artists

The forms below are for use by new and existing body artists/establishments in Cattaraugus County.

The "CCHD: Application for a Certificate to Operate a Tattoo Establishment" is used by new tattoo and permanent makeup establishments who do not already have a "Certificate of Sanitation."

The "CCHD: Consent Form for the Body Piercing of a Minor" is used by body artists to document parental consent when piercing minors.  It is illegal to pierce a minor without parental consent.

Environmental Health Services Fees

All fees charged for Environmental Health services are established by the Cattaraugus County Legislature through local law.

The download below is the most recent revision of the local law establishing fees to be charged by the Cattaraugus County Health Department.  This law became effective August 1, 2011. 

Please note that with the establishment of this fee schedule, non-profit organizations are no longer exempt from fees charged for Environmental Health services.

Sewage Disposal System Permit Application & Information

Construction of small sewage disposal systems in Cattaraugus County requires a permit issued by the Cattaraugus County Health Department.

The "Application for a Small Sewage Disposal System Construction Permit" is the standard form used to obtain a permit for all sewage disposal system types. Read more

Application Form for Building & Zoning Permit in the Town of Olean

The attached PDF contains the Building & Zoning Permit Application Forms for the Town of Olean, New York. Please also download and print the form BP-1 from the New York State Workers' Compensation Board which will need to be filled out as part of the Building & Zoning Application.
