Indoor Tanning Facilities

Effective August 15, 2012, New York State Public Health Law prohibits the use of ultraviolet tanning facilities by children 16 and under.  Additionally, patrons 17-18 years of age require parental consent to use these facilities.

All tanning facilities in New York State using ultraviolet radiation devices are required to obtain a permit to operate.  However, the Cattaraugus County Health Department does not currently have an indoor tanning program.  Tanning facilities in Cattaraugus County are permitted and regulated by the New York State Department of Health. Operation Report Forms for Public Water Systems & Public Pools

The “DOH-360CUV: Water System Operation Report” form is used to report on the daily operation of disinfection systems for public water supplies.

The "DOH-1323: Report on Operation of Swimming Pool" form is used to report on the daily operation of disinfection systems for public swimming pools.

Both of these forms are available in two file types below.  One is a pdf version that may be printed and filled in by hand.  The other is a MS Excel file, for operators that wish to fill out the report on a computer.  Read more
