Long Term Home Health Care (Nursing Home Without Walls)

• The Long Term Home Health Care Program is an alternative to nursing home placement for people of all ages who have a chronic disease or are disabled. Individuals in this program are not expected to make a full recovery and will need ongoing care.
• Services are primarily covered by Medicaid. 
• Potential services include home delivered meals, personal emergency response systems and social daycare, in addition to nursing, therapy and aid services.
