The New York State Vaccination Program prioritizes the distribution of the vaccine by population risk and essential worker phases. We are currently in Phase 1a and 1b of the New York State distribution plan. More individuals will become eligible as the vaccine supply increases

Cattaraugus County Vaccine Clinics
Clinics are NOT open to the general public. If you are a member of Phase 1a or 1b and are currently eligible to receive the vaccine, you may click here to register for your first dose of the vaccine. NOTE: Appointments are subject to vaccine availability!

COVID-19 VACCINE CLINICS are

BY APPOINTMENT ONLY and
Proof of eligibility is REQUIRED. (Any person arriving for vaccination who does not have proof of their occupation or priority status will not be vaccinated. This can include government-issued ID proving age (65 or over), an employee ID card, a letter from an employer or affiliated organization, or a pay stub)
You must complete the New York State COVID-19 Vaccine Form attesting to your eligibility status.

STEPS

1.Go to this website and check eligibility. You will need to bring proof that you meet criteria for Phases 1A and B: https://am-i-eligible.covid19vaccine.health.ny.gov/ (DO NOT use locate providers option to register for vaccination with the Cattaraugus County Health Department)
2.Complete the NYS COVID-19 Vaccine form and bring a copy with you: https://forms.ny.gov/s3/vaccine
3.Go to this link and register for a vaccination appointment with the Cattaraugus County Health Department. You must print a copy of the registration confirmation and bring a copy with you. https://apps.health.ny.gov/doh2/applinks/cdmspr/2/counties?OpID=50501040

Those Who Already Received the Vaccine
Please note that the COVID-19 vaccine requires a second dose 28 days after the first one. The clinics to administer the second dose are currently being planned; anyone who received the first dose from our clinics will receive a notification with registration information in advance of the 28-day mark.

Those who received their first dose are urged to register for V-safe, a smartphone-based tool that provides personalized health check-ins. You can quickly tell CDC if you have any side effects.
