FIRE TRAINING COURSES: I agree to download, print, and have my fire chief sign the State Training Authorization Form that is provided on this website. This form is required by New York State DHSES Office of Fire Prevention and Control for ALL fire training courses. I understand that if I do not bring the completed and signed form to the beginning of first class session, I will not be allowed to participate in skills for that class session. I further understand that if I do not bring the completed and signed form to the beginning of the second class session, that I will be removed from the class and given no credit for attendance. There will no exceptions to this rule.

I also agree to attend all scheduled classes for this course. Students that do not attend all sessions of this course, without the consent of the State Fire Instructor for their absence, may not be given credit for the course and review of their status/ authorization for future classes may be required.

GENERAL (NON FIRE) TRAINING COURSES: I agree to attend all scheduled classes for this course. I understand that I do not have to download, print, and use the State Fire Training Authorization Letter as it is only required for fire training courses.
