FOR FIRE SERVICE TRAINING COURSES:
I agree that I have logged onto the New York State Learning Management System, or LMS, and retrieved my NYS Training ID number from that system. I understand that if I have not logged on to the LMS system and provided my updated training ID that I will not be accepted into the course and will not be allow to begin/ attend class.
Further, I agree to download, print, and have my fire chief sign the State Training Authorization Form provided on this website at the bottom of the page: https://www.cattco.org/oes/training/fire-classes. New York State DHSES Office of Fire Prevention and Control requires that form from each student for ALL fire training courses.
Finally, I agree to attend all scheduled classes for this course. Students that do not attend all sessions of this course, without prior notification and agreement of the State Fire Instructor for their absence, may not be given credit for the course and review of their status/ authorization for future classes may be required.
GENERAL NON-FIRE SERVICE TRAINING COURSES:
I agree to attend all scheduled classes for this course. I understand that I do not have to download, print, and use the State Fire Training Authorization Letter as it is only required for NYS fire training courses. EVOC, TIMS, Highway Safety, and other courses available to EMS providers are still NYS fire training courses and DO REQUIRE the Training Authorization Letter.
